Transcript of Nursing Education (SON)

SNB issues Transcript of Nursing Education as a service to nurses/midwives who were trained locally at the School of Nursing (SON), Ministry of Health, Singapore, prior to its closure in the year 2000.

Application for Transcript of Nursing Education (SON) must be made by the individual nurse/midwife.

How to apply for Transcript of Nursing Education (SON)

1. Download and complete the Application for Transcript of Nursing Education (SON) Form.

2. The SNB Fee for Application for Transcript of Nursing Education (SON) is applicable.

3. You may make the Application by:

a) email with payment made via PayNow to Singapore Nursing Board using our Unique Entity Number (UEN) number - (only applicable to Singapore bank accounts). Please email to SNB@spb.gov.sg for more information about this payment mode.

After SNB has release the PayNow information to you and once you have made payment, please email to SNB@spb.gov.sg with a screenshot of the payment transaction to facilitate for payment verification. Payment verification will take 3 to 5 working days and you will be informed should payment be unsuccessful.

Your application will only be processed after SNB has received the correct payment amount and the signed and completed Application Form. Incomplete Form will result in the delay of your application.

Your request will be sent directly to the nursing regulatory body/educational institution (as provided in your Application Form) by registered mail.

Please note that your request can only be processed if we are able to trace your transcript of nursing education.
